Timed Sexual Intercourse (TSI) is about advising on the best time for intercourse to have the highest probability of conceiving.

Timed Sexual Intercourse is advice to couples if the sperm analysis of the male is normal and there is no ovarian dysfunction or obstructions in fallopian tubes.

TSI treatment can be done without the stimulation of medication by simply monitoring the ovulation cycle of the female to know the time of ovulation for timely intercourse. However, sometimes, ovulation has to be stimulated using hormonal injections, this process is known as Ovulation Induction.

Ovulation Induction: If TSI is done using medication, the procedure begins with taking tablets or hormonal injections that help the eggs to mature during the menstrual cycle. Monitoring scans & blood tests are conducted throughout the menstrual cycle to keep a track of how the body is responding and reduce the risk of multiple pregnancies.

When the body is ready, a ‘trigger’ medication may be given that enhances the ovulation process. The couple is then advised to have sexual intercourse 24-48 hours later when the female body is at its fertile peek.

Cycle Monitoring: This is done using ultrasound or LH kits with or without hormone testing.

On day2 of menses: Baseline scan telling about the ovarian reserve, presence of cysts, shedding off the uterine lining is done.

Subsequent cans are done depending upon the type of stimulation used. With tablets to form eggs the next scan may be scheduled on day 10 of the cycle and following scans on 12th, 14th day however in women where in injections are used for egg formation scans are done more frequently starting from day 6 of the cycle.

The scan monitors the growth of the follicle containing the egg and the uterine lining.

The follicle grows by 2mm per day and the lining by 1mm per day and once the optimal size of 18-20 mm is achieved final trigger injection to release the egg is given and thereafter the intercourse/ Intrauterine insemination/ egg retrieval in IVF is planned accordingly.
