Fertility Tests for Women: A Practical Guide
What Fertility Testing Actually Covers?
Deciding to book your first fertility workup can feel bigger than the actual workup. The tests themselves are usually quick and inexpensive, and a structured fertility assessment for women gives real answers within one or two menstrual cycles.
Fertility Tests for Women fall into three broad groups: hormones (blood tests), anatomy (imaging), and function (ovulation confirmation). Fertility Tests Before IVF are the same as tests for female infertility more generally. There is no separate “before IVF” panel: the workup is the workup, and the results guide whether IVF is even needed. Your partner is tested at the same time. That part is worth stating clearly.
The Core Blood Tests
AMH: Ovarian Reserve
AMH (anti-mullerian hormone) is a blood test that reflects how many eggs remain in your ovaries. Can be done any day of your cycle. No fasting needed. Cost in Delhi: roughly 1,500 to 2,500 rupees at private labs.
Day 2 to 5 Hormone Panel
Blood test done in the first 5 days of your period: FSH, LH, oestradiol, TSH (thyroid), prolactin. Gives a picture of the hormonal environment your cycle starts from. Full panel typically 2,000 to 8,000 rupees depending on inclusions.
Mid-Luteal Progesterone (the Ovulation Blood Test)
An ovulation blood test measured about 7 days after suspected ovulation. Progesterone above 10 ng/mL confirms ovulation happened. Cost roughly 400 to 800 rupees.
Other Blood Tests as Needed
Vitamin D (if deficiency suspected), HbA1c (for insulin resistance and PCOS workup), thyroid autoantibodies where thyroid is abnormal. These are added based on findings, not routinely.
Other tests done routinely to see if you are fit to canary a pregnancy would include: hemogram, hemoglobin electrophoresis to rule out thalassemia, infection testing ( HIV, HbsAg, Anti HCV antibody, VDRL) antibody testing against rubella and chicken pox.
The Imaging Tests
Transvaginal Ultrasound With Antral Follicle Count
Ultrasound probe placed inside the vagina to view the uterus, ovaries, and count antral follicles (small resting follicles in the ovary). Done early in the cycle. Cost 1,500 to 2,500 rupees. Non-invasive, no preparation needed.
HSG or Fallopian Tube Dye Test
A small amount of dye is passed through the cervix into the uterus, and an X-ray shows whether your fallopian tubes are open and whether the uterine cavity is normally shaped. Done after your period ends and before ovulation, roughly day 5 to 10 of your cycle. Cost 3,000 to 8,000 rupees. Most women experience mild to moderate cramping during the dye injection, settling within an hour. A painkiller taken 30 minutes before the test helps.
Hysteroscopy
A small camera inserted through the cervix directly visualises the uterine cavity. Used when HSG shows abnormality or when unexplained infertility persists. Cost 35,000 to 40,000 rupees. Usually done as day care with mild sedation.
Laparoscopy
Only used when endometriosis is strongly suspected or other imaging is inconclusive. Small surgical procedure under general anaesthesia. Not part of the routine workup for most patients.

When in Your Cycle Each Test Is Done
Timing matters for some tests, not others. A rough schedule:
- Day 2 to 5: Baseline hormones (FSH, LH, oestradiol, TSH, prolactin), pelvic ultrasound with antral follicle count.
- Any Day:
- Day 5 to 10 (After Period, Before Ovulation): HSG or fallopian tube dye test.
- Day 20 to 22 (or 7 Days After Ovulation): Mid-luteal progesterone (ovulation blood test).
- Alongside All of the Above: Semen analysis for the male partner.
Most women complete the full workup within one to two menstrual cycles. Your fertility doctor will schedule the tests based on where you are in your current cycle.
The Male Partner Workup: Do Not Skip This
Roughly half of all fertility difficulty involves a male factor. Yet in India, testing the husband is often delayed or skipped entirely. This is one of the most costly mistakes couples make.
Semen analysis is quick, inexpensive (500 to 1,500 rupees), and non-invasive. A single semen sample is analysed within hours for count, motility, morphology, and volume. Some couples need a repeat sample if the first is borderline. Sperm DNA fragmentation testing (roughly 4,000 to 8,000 rupees) is added where the initial semen analysis is normal but pregnancy is not happening, or where there have been recurrent losses.
The workup starts with both partners at the first visit. Full stop.
Complete Fertility Checkup: Total Cost and Value
A complete fertility checkup for both partners at a private centre in Delhi typically costs 10,000 to 25,000 rupees in total (fertility panel cost India varies substantially by clinic). Government hospitals cost 2,000 to 5,000 rupees but usually have longer wait times.
Best-value combination for those wanting to save: use a reputable national lab (Thyrocare, Dr Lal, SRL) for blood tests, and a specialist fertility centre for imaging and interpretation. Always ask for an itemised estimate before proceeding. Bundled “complete fertility checkup” packages can be good value, but check what is and is not included, especially HSG.
Also avoid getting a semen sample from regular path labs, as most technicians are untrained to correctly report the morphology of the sperms. Some labs use automated analysers that can serve as screening for abnormal semen parameters but definitive confirmation will be obtained only if the sample is reanalysed by a trained embryologist.
What Happens After the Tests?
A good clinic returns you for a proper results consultation, explaining what each test shows and what it means for your specific plan. Treatment recommendations should follow findings, not lead them. Depending on findings, you may be pointed to condition-specific care like Low AMH and Pregnancy, Blocked Fallopian Tubes and Pregnancy, Thin Uterine Lining and Fertility
Tests for female infertility are not the destination. They are the starting point. Female Infertility Treatment in Delhi is available at various price points. Fertility Tests Before IVF give your IVF doctor in Delhi the information needed to plan the right treatment for you, whether that is lifestyle intervention, ovulation induction, IUI, or IVF.
Practical FAQ: Fertility Tests for Women
AMH does not require fasting. Fasting glucose and HbA1c do. Ask the lab beforehand as protocols vary. Most fertility hormone tests are not fasting-dependent.
This is the mid-luteal progesterone test, done about 7 days after suspected ovulation. Progesterone above 10 ng/mL confirms ovulation happened. It is the objective confirmation that an ovulation predictor kit alone cannot give.
Partly. Blood tests can be done in one morning. But HSG needs to be timed early in your cycle after your period. So plan for two visits within one cycle for a complete workup.
Most women feel cramping similar to strong period pain for a few minutes during dye injection, settling within 30 minutes. A painkiller taken 30 minutes before helps meaningfully. Some women feel almost nothing. Speak to your doctor about what to expect.
